I just had a customer with the same issue (this with the leave mail on server option checked), but nothing fixed it until I manually edited his mailbox incrementing all message UIDL's. For some reason outlook exress thought it had downloaded messages with those UIDL's, so it skipped them. Fortunately this customer only had 17 messages in his inbox, so it wasn't too difficult. I'm not sure why, but I think something caused Imail to reuse some UIDL's for his box.
