You edited the messages in the mailbox on the server? Is that the X-UIDL header?

I just had a customer with the same issue (this with the leave mail on
server option checked), but nothing fixed it until I manually edited his
mailbox incrementing all message UIDL's. For some reason outlook exress
thought it had downloaded messages with those UIDL's, so it skipped
them. Fortunately this customer only had 17 messages in his inbox, so
it wasn't too difficult. I'm not sure why, but I think something caused
Imail to reuse some UIDL's for his box.

Ok, I know I can't be the only one to have clients whose Outlook client looses sync with the server. Does anyone know how to fix this? My customer says that Outlook isn't pulling down new mail, but he can see new messages when he checks WebMail. Anyone know how to fix this?
