Sounds like they may be "popping" their account to frequently (especially dial-up). I have several users (lusers) that think e-mail is "instant messaging" and have set their client to "pop" their account every minute. If they receive an attachment or message that hasn't finished downloading before the next "pop" interval, the command that the client sends to the server that acknowledges that they have downloaded the messages gets interrupted and will cause the server to download the messages to the client again.
