Upon startup the queue manager reads through the spool directory to find all the messages that are waiting for delivery. Also, during a "queue run", the queue manager will look through the directory to see if any new files have shown up that it didn't know about (it normally will not find any). When there are thousands of files in the directory, that can make the queue manager spend a large amount of time looking through the directory. In version 8.0 this could really slow down the queue manager, but 8.01 made changes which addressed that issue.
