I've seen the problem you describe a few times. Each time, it was the result of antivirus software configured to scan on access in the c:\imail directory.
For example, if using the NAI products, simply exluding the 'outbound' checks is not enough. You must exclude all scanning within the c:\imail\ folder and below.
