As Scott has noted, this is a very complex issue.
perfection is the enemy of the good.
Most IMGate admins block attachments as a first-line of defense. Probably 90+% is blocked (and in case of infection storms, received and DISCARDed silently, not rejected back to the forged envolope sender). This is the "good".
To get "better" with pretensions of perfection, you still have to run an A-V scanner after the attachment rules stage.
