There is no CDMA phone with the 4S.  It is all just one phone.

Since there is only one phone now, we have to change our point of reference.

I would expect the CDMA part to be "hard coded" to Verizon or Sprint.  But it 
still has WCDMA/GSM capability.  I would expect this to be locked to Sprint or 
Verizon as well, the same way it is to AT&T.  Verizon has SIM cards for those 
roaming and I think LTE phones also use SIM cards so they have them for them.
