M means master, S means slave.
+ means messages added, * means flags updated, # means messages trashed
(so it's always zero unless you use Trash).
the second number in each pair is the number of messages for which IMAP
commands were sent, while the first number is the number of messages for
which the IMAP commands already completed.

normally not. it would break down if you synced the same maildir to
multiple servers. other than that, the default is just chosen the way
that works for all mailbox format pairings.
