Thanks for that, although the problem has got more complicated now. I created the image in Photoshop 5.5, so I've gone back and tried all sorts of variations on the jpeg saving options, but there doesn't seem to be a way of saving a cmyk jpeg from Photoshop that Acrobat can read.
I will continue following that up and see what happens.
