This sounds exactly like the problem I had.  The
toggling of good and bad audio was unpredictable,
probably about 10% or less of the time in my case. It
was resolved for me in this thread on the devel list:

In my case, two things were necessary to fix it:
   - the fix available in ivtv 0.4.5
   - the latest beta firmware for the cx25840 chip
One or the other of those alone did not fix it.
BOTH were necessary.

I think mine may be an isolated (lucky) case, because
several other people have reported that they still
have the problem.  I can report that I have been
completely free of tinny audio for about a month.
My stable and working system is:  PVR-500, Debian
stable, kernel 2.6.15.1, ivtv 0.4.5, mythtv 0.19

Are you using the latest firmware?  Your log should
have something like this:
cx25840 1-0044: loaded v4l-cx25840.fw firmware (16382
bytes)

If it says something other than "16382 bytes", then
you are not using the latest HcwMakoC.ROM.
Although I never comfirmed how necessary it was, I was
extra careful to do a complete power down to "reset"
the pvr card after the new firmware was loaded the
first time.
