I was getting this behavior upgrading to the latest firmware.  Sorry, 
don't know exactly what it is, but I followed directions from one of the 
latest "tinny audio" posts on this forum to upgrade to the most recent 
firmware.

After the upgrade, ivtvctl -q 0 ALWAYS toggled bad audio to good.
