What's up with the "off" line?  I'm not sure that will work.  If  
you're trying to force the PVR-500 onto video1 and video2, do this :

alias char-major-81 ivtv
alias char-major-81-1 ivtv
alias char-major-81-2 ivtv
options ivtv ivtv_first_minor=1

If you look through the dmesg output, you'll see that the first card  
detected is the second unit of the PVR-500, not the first.
