I've just looked at the dialog. There are two checkboxes followed by a combo box. Like you, I have the two checkboxes checked. For the combo box, I have "always" selected. JAWS starts at the log-in screen on my system. Is it possible you have made a different selection in this combo box?
Also, for the sake of clarification, you understand that JAWS speaks only enough to get you past log-in. It still takes several moments (less than a minute) for JAWS to come up speaking in a usable way. I'm making sure all possibilities have been covered.
