If your router/nat supports it you could redirect based on port.
Like:

NAT IP port 10161 -> Server1 Port 161
NAT IP port 10162 -> Server2 Port 161
NAT IP port 10163 -> Server3 Port 161

And then monitor the server in jffnms as "NAT_IP:10163"
