While reading a message or inside an open message use insert+a to see if
jaws announces whether or not attachments are found to this message.
Next, press the enter key to open the attachment.  If it is a text file, you
can probably read it.
If it is a sound clip or video file or audio file, you will probably hear
nothing, because to do so, you would have to switch jaws off, unload it,
before you press the enter key when opening an audio or video file.
When you have completed reading or listening to a file just close it with
alt+F4 and you are back in the message body.

Then reload jaws for continued screen reading.
