Jaws and Screen Reader friendly personal communications software

How difficult is it to use something like ICQ, to set up and to 
use.  I use ICQ for supporting clients and its great I consider
it like working in the next office even though you can be
thousands of miles away.  That would be my first choose
for supporting the programs I have been working on,
we do have a list server but it just isn't real time enough
to be anything but frustrating sometimes.

Even a regular old chat would be ok, as long as it is 
easily accessible.

Any ideas??
