On Windows 7:
FF = loads applet and model
Chrome = Loads blank applet only
MSIE = nada - blank page.
All latest browser versions.

If you read the pages from a server, yes, any folder arrangement is
fine. Or that's what they say, I always work local.
2.
You could easily have a timing issue with that setup, but I don't
think it's the case. The error says clearly it's a file access problem.
3.
First, are you sure you have cleared the browser and Java cache?
That gives a lot of trouble like the one you have.
I can't imagine anything else.
Do you have a url I can test?
