No charge for the card.
I do not want to have a checkout process.
They also have to be able to select the card.

Are you wanting to charge them for the card? If that's the case you
could roll with QuickCart from Rocket Theme:

If not, then you're just submitting a form and capturing it in
database right? Could use any of the form extensions to do that easily
enough. In situations like that I usually write a cronjob that fires
off a CSV export to an email address for human processing.
