There are two ways to store georeferencing into PDF. One is Adobe Supplement to 
the PDF standard ISO 32000, another is a system developed by TerraGo company 
but it is published as an open Best Practice paper by OGC.  GDAL supports them 
both and links to corresponding original documents can be found from the PDF 
driver page http://www.gdal.org/frmt_pdf.html.
In practice geospatial PDF means that geospatially enabled viewer can show the 
coordinates from any point on the map, possibly in many coordinate systems. In 
other way, viewer can take coordinates as an input and show the place. At least 
one viewer can even take the coordinates from GPS.  GDAL PDF page

I do not really understand how and where in the PDF file GDAL stores the 
attributes. They appear under the "Model tree" tab in Adobe Reader and system 
seems to utilize somehow the 3-dimensional features of PDF documents. For sure 
this is described in the 1310 pages long standard

I would guess that a faster way would be to ask directly from Even Rouault, the 
GDAL maintainer.
