No, startkde still canonical way to launch KDE (whether in .xinitrc or 
otherwise). I'm not even sure what 'kdeinitrc' is, I surely don't have one.

Not sure why that's happening, works for me. Might be an issue with X and your 
graphics hardware.

Can be safely ignored.

You can start multiple X sessions the usual way, e.g. startx -- :1, startx

Configure multiple screens in X. How to do that is out of the scope of this 
mailing list, but there's lots of information available on that subject on 
the net, look for keywords xorg, xfree86, dualhead, multihead and xinerama.

Control Center/Appearance & Themes/Colors (Widget Types: Active/Inactive Title 
Bar, Active/Inactive Title Text). It's not possible to override window 
titles.
