Back in the day when I'd rip video off Tivo I had good luck with Q-DvdAuthor:

It (as well as just about any DVD menu/authoring tool) has some prerequisites, as well as QT, but it was pretty solid for mastering simple DVD's from pre-existing streams.

I don't recall if Q-DvdAuthor has a facility to add chapter marks to your streams, but at the time I used GopChop (http://outflux.net/unix/software/GOPchop/) to cut out the commercials, at which time a chapter marker was automatically added.
