Help -> Help Contents -> Customizing Firefox -> Getting and Installing Add-Ons
gives some clues.

Search the Knowledge Base  "unable to install add-ons" and look at the
first search result.  Read down the page until you get to "You are
asked to download the add-on, rather than installing it"
