His comments were my exact thoughts.  It's not like your "quality of work" to not have that bridge piece painted and polished!

Others mentioned two of my thoughts:
1. Put Locktite on the threads to deter the nut from backing off.
2. They asked about that gear in relation to its original position in the gear train vs your "extended gear train".  I was thinking that its original (OEM) position was for it to be positioned such that its primary (maybe only?) rotational direction was one that would tend to tighten the axle screw - vs loosen it, as you are experiencing.

Another thought was to replace the axle screw with a reverse thread one.  I realize that's too much trouble - but thought "Heck - Joe'd do it!

I think your easiest fix was Locktite - but you did not let us down.

You came up with your "Paul Bunyan" fix - and it works! Congrats! Mission Accomplished!
