In this instance, since there was not one name always used for your daughter, I would enter her as Kathryn Andrea Martell - no quotes anywhere. Then, either in General Notes, or in AKA (depending on your personal preference) I would list Kathy, Kate and Kath. The reason for using quotation marks around a 'generally known by' name is so that reports read freely - e.g. My name is Janet, but I am RARELY called that, so it sounds 'wrong' for reports to read - Janet did this, Janet did that - much more natural to read Jan did this, Jan did that. However, my correct, full name is still recorded for all to see.
