Jenny, I haven't got time to check this out at the moment, so I'm going, off memory. I suspect what Michele may be doing is to enter the death then the the source with child 1 in the detail, then clicking the blue line agin and entering child2 and repeating. I think in this way only one source will be recorded.
If it is first saved before entering the second and saved each time before repeating I suspect it will be ok.
