Mark, A few states have "Independent Cities" that are not located in any Counties. Here is a list of a few, all in Virginia since I am well aware of these.

The whole US is divided into states which is divided into counties. I know of no situation where there is a place that is in "no-man's land", that is just uneconomically unviable in any first world environment.
