Alexandria and Richmond, cities in the State of Virginia, and Baltimore in the 
State of Maryland are not in any county.  They are independent cities.  I 
believe that there are others too.  There is also Washington, D.C. which is not 
in a state.

I'm curious what it means to be "uneconomically unviable in any first world 
environment".

I am sorry, but that just simply does not make sense. The whole US is divided 
into states which is divided into counties. I know of no situation where there 
is a place that is in "no-man's land", that is just uneconomically unviable in 
any first world environment.

Please enlighten us how a US city cannot be in a county, I really do want to 
know.
