Bill,

In your situation, it doesn't make sense.

I too have a number of relatives who go by their middle name instead of
their first name.

My mother has always gone by her middle name, because she was named after my
grandmother's middle name and my grandmother was already using her middle
name because she was named after her mother.
Great Grandmother: Charlotte Marion - went by Charlotte
Grandmother: Charlotte Virginia - went by Virginia
Mother: Virginia Lynn - goes by Lynn

I also have several male relatives that all have the same first name, and
therefore used their middle name to distinguish themselves from their
father, grandfather, uncle or cousin.

I have played around with using the "quoted" names, the AKAs and also used
an Event called "Nickname" and I haven't come up with a preferred method. I
like having the "quoted" name because it will be used in reports, but I
don't like the way it shows up on the charts and how it exports to GEDCOMs.
It would be nice is we could have a "Preferred Name" option so that we could
display names by the "preferred name" but also have the actual/birth name
recorded as well.

Sorry, but do not agree with your concerns IMHO.

My father's first & middle name was Paul King and only on legal documents
and his mother called him Paul. To everyone else, P.K. was used. Think his
official record in Legacy should be Paul King and AKA as PK which was
reality.
