Some commands like \pad-around are nearly useless at the moment, so I
suggest this is Critical.
The pedestrian solution seems the most clear:
Have \pad-around and similar insert a stencil-expression that Mike's
skylining can see.
Have \with-dimensions tag the real stencil so Mike's skylining can ignore it
If this concept meets the need, I should go back and implement a specific
stencil-expression that means 'reserve space in skylines' but does not put
anything in the PostScript output.
