You can centre text above a bar line using a rehearsal mark; see http://lilypond.org/doc/v2.15/Documentation/notation/bars#rehearsal-marks
As to the issue with the rest: it seems to me that lilypond is doing the right thing here: a rest _is_ just the rectangle: the line comes from a ledger line.
