Because Windows is not case sensitive and Unix is. Very easy to fat-finger a filename. I've seen it on my own website when switching from a Windows based server to a Unix based server on a hyperlink to a JPG, for example where the uploaded JPG has a different case to the hyperlink (works under Windows but not Unix).
