After months of struggling with the HPT366[1], I gave up and installed a
PDC20262. Now all my devices work perfectly. Even my cdrw and 40x cdrom
can now run in udma mode.

The HPT366 is just broken, broken, broken.

The $30 spent on PDC20262 controller was definitely worth it, to get rid
of these stupid problems.

[1] Aside from the random lockups even in PIO mode, the HPT366 was never
able to run any cdroms in DMA mode. If DMA (even standard DMA, not
even UDMA) was enabled on any cdrom, it would lock up instantly on any
    access to the cdrom device.
