Add the following:
 utility:[EMAIL PROTECTED]
 utility:server:
         :rw:sf:sh:
                 :lp=utility@printer:
         :sd=/var/spool/lpd/%P:
         :lf=lpd-errs:
         :mx=0
         :lpd_bounce:

Now I can tell you that part of the problem will be solved -
you will now be spooling instead of sending directly to the printer.
This may also solve some mysterious 'lost job' problems.

Next, check to see that the PRINTER is not generating banners.
I bet that it is.
