I do not know which proxy software you are using but I have in the past turned
on delay pools with squid.  I split network users into groups and could
specify how much bandwidth was allocated to a pool as well as the maximum for
each user in each pool.  It seemed to work pretty well although I did not get
round to really measuring on a per host basis.. that could be an interesting
next step...
