Hi Glenn.

In iTunes for Windows, you get a Sources List.
If you navigate right to the top to where it says Music, then, arrow down to 
Apps.
then tab until you get the Apps tab.e
Select that app, then press the Backspace key on your keyboard.
It will ask you if you wish to delete it, delete it without removing it from 
the computer, or Cancel.
Press enter on Delete, and it should be gone.
