Don't know what else to tell you then except to reinstall the Itools again
and to go to the Iomega site to get the latest update. Quick
question....are those ID #'s shown as SCSI ID's?  Are all the connections
tight?  I wonder since you say that one time the SCSI Probe sees the drive
and not other times..I have found that sometimes the ac connection will
provide enough power to run the green light but not enough to operate the
drive so it can be seen by the rest of the system..  Do you have anything
else connected to the cpu?  It took me a while to get it set properly when
I initially set it up with my Performa 5200 and then set it up on the imac
with the SCSI/USB adapter.. As a last resort you can always call Iomega
Tech Support for help...
