As long as Configure says:

    Perl can be built to take advantage of 64-bit integer types
on some systems. To do so, Configure can be run with - Duse64bitint. Choosing this option will most probably introduce binary incompatibilities.

it is unlikely we would be switching, if we risk breaking people.

And I've only gotten one request for it over the last few years, so it's not high on my list.
