I sincerely hope you come up with a better solution for this problem than I did about a month ago when, first MBraille, then Apple Braille Screen input exhibited the symptoms you describe for no apparent reason. I ended up using the reset all settings to fix it, and it hasn't gone wrong since. Warning! If you have to go this route, you will have to get sighted help to turn VoiceOver back on. The triple click home button won't do it, and serie is turned off, so it can't turn it on, either. But reset all settings will fix it. I hope you find a less drastic fix. If it is like mine was, and it sounds like it, you are not the problem, something has gotten bungled up in the settings for braille screen input.
