      Yesterday it happened to my iPhone 11 Pro. As I tried to switch it off 
and on again somehow the emergency call became activated and all my emergency 
contacts got a message. I didn't notice. So you can imagine what a mess.

      Actually Apple should have warned VO users about this massiv bug.

        Yes when you plug the phone in to your computer it should prompt you 
for the phone to be unlocked which you have to do on the phone not in itunes,

        Once the phone is unlocked itunes will give you what you require.

iTunes would need the passcode, that I cannot insert without sound. Did they

        Hi,

        Sounds like there'll need to be some console log reports so that things 
can be figured out.  If VO speech is stopping due to some sort of conflict, 
then they'll need to figure out what processes are conflicting that renders VO 
speech inoperable.  If it is more widespread than just to do with the phone 
app, then workarounds aren't the fix, it will need to be a bug fix for sure.

        I've only had this happen once.  A full DFU is not required at all, and 
if anyone claims that it is necessary, they're wrong.  A simple restart will 
bring things back to normal.  If you haptic feedback on, then it is easier than 
with it off. While haptic feedback is on, press the down volume and the side 
button in for a few seconds until you feel the haptic feedback. This has 
brought up the shutdown dialog.  Be careful here, as you could accidentally 
call emergency services if you have that feature enabled.  What you do is to 
flick left until the haptic feedback feels like you're hitting the left wall.  
When you're at this point, you are on the "Power Down" button, so double-tap 
and your iPhone will turn itself off.  Wait about a minute between 
double-tapping to shutdown, and holding the side button to turn the iPhone back 
on.  Once it restarts fully, VO will be working normally again.

        If VO is off, then flicking left will not result in the haptic feedback 
hitting the left wall.  So, you'd then need to press the side button three 
times to turn VO on, then follow the steps above to turn the iPhone off and 
back on.  I haven't totally figured out what the trigger is that causes this to 
happen, but I believe it has something to do with the screen going to sleep 
while on the call.  If it's a shorter call where the screen does not put itself 
to sleep, then this does not usually happen.  At least in my situation, that's 
been the case.

        How to report such a problem to Apple? Sometimes I feel like at the 
call centre thy are trying more to say  that it is normal than trying to 
understand the issue. Also, they will ask for a full restore with DFU mode… I 
can see this happening…

        Your not going crazy mine has done it a few times also. Along with
