Yes, there should be a crush washer there. Another thing I thought of: If banjo bolts are over torqued, the stretch and is yours is over stretched, that could cause problems. If the holes in the side of the bolts look oval, replace the bolts. I ran into this on 123 240D auto trans oil line connections. I think your biggest need is simply for the crush washer.
