Because if they don't keep the concrete agitating (mixing) it will set up 
faster.  Once it sets up, you're done.  Or for that matter, the truck is done.

Also, due to the arrangement of the mixing vanes in the drum, depending on the 
direction of rotation it can mix the concrete or force it out for delivery.
