Commentary there says the woman WAS wearing a seatbelt, and the shrapnel WAS contained.
But... A seatbelt can only do so much, and the window WAS right there next to her. Also, the fan shrapnel was contained, but the horrendous vibration caused other parts of the engine nacelle to fracture and fragment into the 500MPH slipstream. So, not "shrapnel" damage so much as "debris" damage, not that it matters to the injured/killed. Also, behind the wing, not on the rotational axis of the turbine.
