Normally the transmission sits in second gear at idle and stationary. It downshifts to first when you step on the accelerator any amount. When its adjusted properly, you never even notice. This is to reduce creep. Since you are starting in second gear and shifting early, It seems very likely its the cable. That is, the transmission doesn't know you're accelerating. In L, it will hold first gear unit redline no matter how hard you press the accelerator. If you move it to S and back to L quickly, you'll be in second at that point. Shift back up to S and you'll be in third. And D goes to forth if you're going fast enough. Try winding out each gear like this to ensure you can distinctly feel each one. Its very likely you're not getting into 4th and you drove without any issues at 70mph for a long period of time.
