Windows: double.
Two double pane windows, one on the outside of the wall and one on the
inside. Awesome insulation and very good sound attenuation.

Walls: acoustic insulation.
Sprayed in place foam preferred, blown in cellulose good enough.
For bonus points in new construction, make a 6" wall with staggered 2x4s,
so that the exterior sheathing and interior drywall aren't attached to the
same hunk of wood.
