Starting in 1st is correct, the pressure cable was slightly misadjusted causing it not to downshift without hitting the pedal.

Mine started in 1st, but I don't think it was adjusted quite right.  For
example, if I was coasting and shifted from D to L, it wouldn't actually
downshift from 4th to 3rd unless I tapped the accelerator.  So I may
have been seeing abnormal behavior.
