You can't configure MNI drivers with MiamiInit.
When configuring manually, make sure you do the "Query Device" as part of
the setup, and accept the results, or the packet types won't be set
correctly.

You can't do much of anything until the interface is considered online.
If you use a static setup while disabling everything that might need
network responses, you should be able to get "online" and use tools like
MiamiPing and MiamiTCPDump to investigate further. The new MiamiTCPDump
with MiamiDx even has a way to see the "going online" traffic, but I doubt
that it works with Miami.
