It's possible that there's some odd configuration in the modem that
doesn't cause trouble after MiamiInit has screwed around with it, but
isn't fixed by the init string. Do you also see this trouble after
power-cycling the modem?
A couple of things you might try are:
0) See below.
1) Checking the modem config with a comm program in both the good and bad
cases. IIRC Supra uses AT&V to list the settings, but you should check
the manual.
2) Resetting the modem to factory defaults and then rerunning MiamiInit.

Sort of. If it were literally a "reply" there wouldn't be an issue, but
the problem is that *each* end initiates negotiations with the other, and
mere echoing could be mistaken for proper server activity. Thus, the
magic number is included to distinguish between the two parties. This
message normally means that the connection is merely echoing the output
back to the input. The modem itself will typically do this when it's not
online.

Sounds like you have "Use CD" enabled and CD is being asserted all the
time, either due to a modem misconfiguration or a miswired cable. Make
sure the modem is configured for AT&C1, as AT&C0 could cause a symptom
like this. I think the CD check is too early for putting AT&C1 in the
init string to help. You could turn off "Use CD", but it's better to have
CD work properly.

If you got that message it thought it was already in the PPP stage, and
thus wouldn't have dialed.
