This mostly happens as a result of a window.open() in an onclose()
handler ( I think that's the right name. )

Could we add a pref'ed OK/Cancel question dialog when that happens?  It
happens so rarely and I know a lot of unsuspecting people that has
happened to.  Of course, I don't know if it's possible to tell if you
are doing an open in a close.
