So basically this: If a chapter, as written in the booklet, spans multiple tracks, simply use the chaptername for every track. If the chapters are numbered, use this. No numbering at all. Special cases left aside, but I'm pretty sure the above covers at least 80% of all releases on CD. Or did I miss anything?
