So basically this:
If a chapter, as written in the booklet, spans multiple tracks, simply
use the chaptername for every track.
If the chapters are numbered, use this.
No numbering at all.
Special cases left aside, but I'm pretty sure the above covers at least
80% of all releases on CD.
Or did I miss anything?

BTW, Commercial DBs like Gracenote use roman numbers for numbering a
chapter that spans multiple tracks (i.e. I,IV,X, etc)
