FWIW, I've been using mutt on OS X for some time....works perfectly.

I have set my aliases (and other sourced files) as in the example below
set alias_file=~/Library/Preferences/mutt/aliases

...


source ~/Library/Preferences/mutt/aliases
source ~/Library/Preferences/mutt/gpg.rc
source ~/Library/Preferences/mutt/send-hooks
source ~/Library/Preferences/mutt/save-hooks
source ~/Library/Preferences/mutt/macros

all of these lines are in the .muttrc file.

A note for non- OSX users... ~/Library/ is the canonical place for users
pref files (Library/Preferences) and configuration files. I like this as
it keeps ~ a bit tidier....
